Requirements

Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.
Licensed to practice as a physical therapist in the state of practice.
Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum of 6-8 weeks.
Must have current Driver’s license and reliable transportation to and from work site.

Nice To Haves

Current working knowledge of conditions of participation and home healthcare experience preferred.

Responsibilities

Provide one-on-one care in patients' homes.
Use cutting-edge tech to help patients stay safe and healthy.
Maintain clinical practice skills.
Learn and adhere to organization’s policies and procedures on an on-going basis.
Function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with agency’s clients and families.
Consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
