About The Position

H2 Health is seeking a Float Physical Therapist to support our outpatient clinics throughout the Floyd, Virginia region. This role offers variety in the workday, collaboration with different teams, and the opportunity to make a meaningful impact across multiple communities. It is suitable for both experienced clinicians and new graduates eager to expand their skills, with support provided through mentorship, professional development, and a collaborative clinical network. As a Float Physical Therapist, you will work with diverse patient populations across multiple outpatient locations in and around the Floyd market, providing individualized, evidence-based care to patients recovering from orthopedic injuries, surgical procedures, balance and vestibular disorders, musculoskeletal impairments, and mobility limitations. This role allows for broadening clinical experience, collaborating with multiple experienced teams, and building strong relationships throughout the communities served.

Requirements

  • Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
  • Current Virginia Physical Therapist license or eligibility for licensure.
  • CPR certification or willingness to obtain as required.
  • Ability and willingness to travel between assigned clinic locations.
  • New graduates are encouraged to apply.

Responsibilities

  • Perform comprehensive patient evaluations and develop individualized plans of care.
  • Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
  • Treat a variety of orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions.
  • Provide exceptional patient care across multiple outpatient clinic locations within the Floyd region.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients on injury prevention, wellness, and home exercise programs.
  • Maintain accurate and timely documentation of patient progress and treatment outcomes.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
