About The Position

Gallagher Home Health Services is hiring a dedicated, experienced Physical Therapist to join our team as an independent contractor covering the New Castle area. This role offers flexibility and independence, allowing you to schedule 10-20 patient visits per week and design your schedule around your life and your patients' needs. As a Home Health PT, you will provide personalized care in the homes of patients, helping them regain mobility and independence. Our patient-first approach allows you to focus on delivering compassionate, high-quality care with autonomy in managing your schedule.

Requirements

• Licensed Physical Therapist in Pennsylvania
• Minimum of 1 year of recent experience required
• Must have a reliable vehicle, valid PA driver’s license, and insurance coverage
• Must pass a satisfactory criminal record check

• Home Health experience preferred

Responsibilities

• Assess and evaluate patients' physical conditions and needs to develop individualized care plans.
• Provide hands-on physical therapy treatments, including exercises and modalities, to enhance patient recovery.
• Document patient progress and maintain accurate records using EMR systems.
• Educate patients and families on treatment plans and home care strategies to promote independence and recovery.
• Drive to various locations for home health visits, ensuring timely and effective care delivery.
