About The Position

Folkston Park Care & Rehab Center is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our in-house rehabilitation team. This position provides physical therapy services to residents in our subacute rehabilitation and skilled nursing setting, helping patients regain mobility, maximize functional independence, and achieve their highest level of recovery. As part of our interdisciplinary team, the Physical Therapist will evaluate, develop, and implement individualized treatment plans for residents recovering from orthopedic surgeries, neurological conditions, cardiac events, generalized debility, and other medically complex diagnoses.

Requirements

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Georgia.
  • Current CPR certification or ability to obtain upon hire.
  • Strong clinical, communication, and documentation skills.
  • Ability to work independently and as part of a multidisciplinary team.

Nice To Haves

  • Experience in skilled nursing, subacute rehabilitation, long-term care, or post-acute settings.
  • Knowledge of Medicare Part A and Part B rehabilitation services.
  • Experience treating orthopedic, neurological, and medically complex patients.

Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Provide evidence-based therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
  • Establish measurable goals and monitor patient progress.
  • Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with regulatory and facility standards.
  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.
  • Educate residents, families, and caregivers on treatment plans and home exercise programs.
  • Participate in interdisciplinary care planning meetings as needed.
  • Ensure compliance with Medicare, Medicaid, state, federal, and facility regulations.
  • Maintain productivity and quality standards while delivering exceptional patient care.
