About The Position

FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Boyertown, PA, to provide exceptional one-on-one care to older adults. This role involves delivering physical therapy services directly in patients' homes or within Senior Living Communities, utilizing FOX's signature Geriatric House Calls™ model. Therapists will help patients improve mobility, independence, and quality of life. The position offers autonomy and flexibility to create and manage treatment schedules, build lasting patient relationships, and incorporate telehealth/virtual care for greater flexibility and continuity. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Physical Therapy program
Basic computer and EMR proficiency
Current CPR certification
Ability to meet the physical demands of in-home and community-based care, including: Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
Performing CPR
Providing moderate to maximum assistance with patient transfers and bed mobility
Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
Wearing PPE when required

Responsibilities

Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
