About The Position

Desert Springs Post Acute is transitioning to Willow Springs Post Acute and is seeking a motivated and compassionate Physical Therapist. This role involves helping patients regain mobility, restore independence, and achieve recovery goals. Requirements

• Current Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• Strong clinical reasoning and assessment skills
• Excellent communication and interpersonal abilities
• Ability to thrive in a collaborative healthcare environment

Nice To Haves

• Skilled nursing or post-acute experience preferred but not required

Responsibilities

• Evaluate patients and establish individualized therapy plans
• Develop functional goals that support recovery and independence
• Implement therapeutic exercise programs and mobility training
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and families regarding treatment goals and home exercise programs
• Collaborate with interdisciplinary team members to support successful patient outcomes
• Complete timely and accurate clinical documentation
• Participate in discharge planning and care conferences
