About The Position

They are seeking a Physical Therapist for their San Francisco County team who desires flexibility, independence, modern tools, and an organized home health structure. The role involves serving patients across diverse neighborhoods in San Francisco, with a caseload designed to minimize drive time and maximize patient impact.

Requirements

• Current California Physical Therapist license
• Valid California driver's license
• Strong clinical judgment, organization, and time-management skills

Nice To Haves

• Home health experience preferred; strong outpatient, inpatient, or rehab PTs are encouraged to apply

Responsibilities

• Independently perform physical therapy evaluations and reassessments
• Develop and manage individualized plans of care
• Deliver skilled therapy focused on mobility, function, fall prevention, and independence
• Collaborate with PTAs on an occasional, well-supported basis
• Communicate directly with physicians and the interdisciplinary team
• Educate patients and caregivers
• Complete documentation supported by AI tools
• Serve as the primary PT voice in patient care planning
