About The Position
Physical Therapist (PT) - Skilled Nursing & Rehabilitation - Full-Time & PRN | Cincinnati, OH Join a Team Focused on Restoring Independence At Concept Rehab, Physical Therapists play a critical role in helping patients rebuild strength, mobility, confidence, and quality of life following illness, injury, surgery, or hospitalization. We are seeking a compassionate and motivated Physical Therapist to join our rehabilitation team in a skilled nursing and post-acute care setting. This position offers the opportunity to work alongside experienced clinicians who are committed to exceptional patient outcomes and collaborative care.
Requirements
- Graduate of an accredited Physical Therapy program
- Current Ohio Physical Therapist license in good standing
- Excellent communication and clinical reasoning skills
- Ability to build strong relationships with patients, families, and healthcare professionals
- New graduates are encouraged to apply
Nice To Haves
- Skilled nursing, post-acute, or geriatric rehabilitation experience preferred
Responsibilities
- Perform comprehensive physical therapy evaluations and establish individualized treatment plans
- Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence
- Monitor patient progress and adjust treatment strategies to maximize outcomes
- Collaborate with physicians, nursing staff, families, and interdisciplinary care teams
- Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations
- Complete timely, accurate, and compliant clinical documentation
- Educate patients and caregivers on safe mobility techniques and discharge recommendations
- Participate in care conferences, team meetings, and quality improvement initiatives
- Promote a positive patient experience through compassionate and patient-centered care
