Physical Therapist - Midland, MI
About The Position
FOX Rehabilitation is expanding in Midland, MI, and nearby areas, seeking licensed Physical Therapists to provide dedicated one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on enhancing patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position is based in two partner Senior Living Communities: Bickford Saginaw and Bickford Midland. The opportunity is suited for Physical Therapists who value autonomy, flexibility, and impactful patient outcomes, with the ability to manage their own schedules and build strong patient relationships. Clinicians may also utilize telehealth and virtual care for increased flexibility and patient support. This is a high-impact PT role aimed at positively influencing the lives of the communities served.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification

Responsibilities
- Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
