Requirements
- Graduate of an accredited Doctor of Physical Therapy (DPT) program.
- Licensed or license-eligible Physical Therapist in the state of practice.
- New graduates are strongly encouraged to apply!
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.
Responsibilities
- Evaluate and assess patients' functional status and mobility.
- Develop individualized treatment plans to maximize independence and safety.
- Provide skilled therapeutic interventions in patients' homes.
- Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Complete timely electronic documentation while maintaining the highest standards of patient care.
