About The Position
Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. Join our team and help older adults maintain their independence and quality of life through compassionate, evidence-based care.
Requirements
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- Active Florida Physical Therapist (PT) license

Responsibilities
- Evaluate and treat residents in an assisted living community and or Home setting
- Develop and implement individualized treatment plans
- Collaborate with physicians, families, and the interdisciplinary care team
- Maintain accurate, timely documentation in the EMR
