About The Position

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in Reisterstown, Maryland, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may incorporate telehealth and other virtual care into their schedules. The practice is clinician-operated and emphasizes meaningful relationships, evidence-based care, interdisciplinary collaboration, clinician-driven decision-making, and a mission to abolish ageism. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time roles are salaried with benefits; part-time roles are salaried with benefits eligibility based on a minimum-hours requirement; PRN/FOX Flex roles are paid per unit. Compensation depends on experience and employment type. In Maryland, the standard full-time base pay range is $70,000–$110,000 annually, with the opportunity to earn more; part-time and PRN compensation is prorated.

Requirements

Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear personal protective equipment when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Travel by car to patient homes and senior living communities.
