About The Position
Momentum PT is a physical therapist-owned outpatient practice seeking a passionate Physical Therapist to join their team in Pueblo, CO. This role offers the opportunity to practice hands-on PT, take ownership of patient care, and develop into a future clinic leader. The practice emphasizes a collaborative and supportive work environment where clinicians have a voice in patient care and clinic culture, leading to nationally ranked patient outcomes. The position is open to Full-time, Part-Time, or PRN candidates, including new graduates.
Requirements
- Doctor of Physical Therapy (DPT) from an accredited program
- Licensed or eligible for licensure in Colorado
- Current CPR certification
- Strong communication and interpersonal skills
- Motivation to grow into leadership opportunities

Responsibilities
- Evaluating patients and creating individualized, evidence-based treatment plans
- Providing interventions such as manual therapy, therapeutic exercise, and specialty modalities
- Educating patients and families on exercises, recovery, and injury prevention
- Tracking progress and adjusting plans as patients advance
- Collaborating with other healthcare providers to ensure comprehensive care
- Maintaining clear documentation in line with HIPAA and state/federal rules using state-of-the-art AI technology
