About The Position

Dove Healthcare is seeking a Physical Therapist to plan, develop, organize, implement, evaluate, and direct physical therapy programs and activities in a skilled nursing facility, adhering to current rules, regulations, and guidelines. The role involves participation in discharge planning, patient care plan development and implementation, patient assessments, and maintaining treatment records, patient files, and progress notes. Collaboration and supervision of Physical Therapist Assistants (PTAs) are also key aspects of the position. Dove Healthcare is a healthcare provider with 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes across 11 Wisconsin communities, committed to providing innovative, compassionate, and high-quality care. Requirements

  • doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
  • current license as a physical therapist in the State of Wisconsin

Responsibilities

  • plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
  • participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
  • maintain treatment records, patient files, and progress notes as required
  • participate in the development and implementation of patient assessments and care plans, including quarterly reviews
  • collaborate and supervise PTAs
