About The Position
Ultimate Therapy is hiring a Physical Therapist (PT) to join our skilled nursing rehabilitation team. We are looking for a therapist who is passionate about helping residents restore mobility, improve confidence, and maximize functional independence through individualized rehab care.
Requirements
- Active Physical Therapist license required
- Strong communication and clinical reasoning skills
Nice To Haves
- Skilled nursing experience preferred but not required
Responsibilities
- Perform physical therapy evaluations and create individualized treatment plans
- Provide therapy focused on mobility, strength, balance, endurance, and pain management
- Work with residents recovering from orthopedic, neurological, and post-acute conditions
- Collaborate with PTA staff, nursing, physicians, and interdisciplinary rehab teams
- Monitor patient progress and adjust treatment approaches as needed
- Maintain timely and accurate documentation
