About The Position

Human Touch Home Health is seeking Physical Therapists (PTs) for PRN home health opportunities throughout Northern Virginia. We have patient needs across Fairfax, Loudoun, Prince William, Arlington, Alexandria, and Falls Church, with opportunities in communities including McLean, Vienna, Annandale, Springfield, Burke, Reston, Herndon, Chantilly, Centreville, Leesburg, Ashburn, Sterling, Manassas, Gainesville, Haymarket, Woodbridge, Dumfries, and surrounding areas. You do NOT have to cover all of Northern Virginia. Choose the cities or areas that work best for you and build your caseload around your availability. Home health gives you the opportunity to provide focused, one-on-one therapy while helping patients regain mobility and independence where it matters most — at home.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky
