About The Position
FOX Rehabilitation is hiring a licensed Physical Therapist to provide one-on-one care to older adults in Torrington, Connecticut, and surrounding communities. Care is delivered in patients’ homes and in senior living settings, including independent living, assisted living, and memory care. Clinicians use FOX’s Geriatric House Calls™ model to help patients improve mobility, independence, wellness, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Physical Therapists collaborate with an interdisciplinary team and educate patients, caregivers, and senior living staff. This posting describes full-time, part-time, and PRN/FOX Flex employment pathways; the PRN/FOX Flex pathway is paid per unit. Benefits are available for eligible roles. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
