About The Position

We are seeking an experienced Physical Therapist to provide outpatient rehabilitation services within a healthcare setting. The ideal candidate will evaluate patients, develop individualized treatment plans, and implement evidence-based therapeutic interventions to improve mobility, function, and overall quality of life. Candidates with acute rehabilitation experience or a willingness to float to rehabilitation units are highly encouraged to apply.

Requirements

Graduate of an accredited Physical Therapy program required.
Active California Physical Therapist License required.
Current BLS Certification required.
Minimum three (3) years of Physical Therapy experience required.
Willingness to float to rehabilitation-related units required.

Nice To Haves

Outpatient rehabilitation experience preferred.
Acute rehabilitation experience preferred.
Previous travel experience preferred but not required.
Experience with electronic health record systems preferred.

Responsibilities

Perform comprehensive physical therapy evaluations and assessments.
Develop individualized treatment plans based on patient needs, goals, and physician referrals.
Implement therapeutic exercise programs, manual therapy techniques, and functional mobility training.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients and caregivers on home exercise programs, injury prevention, and rehabilitation goals.
Maintain accurate and timely clinical documentation in the electronic medical record.
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
Participate in discharge planning and continuity of care activities.
Maintain compliance with regulatory standards, professional practice guidelines, and facility policies.
Float to other rehabilitation-related units or patient care areas as needed.
