About The Position

Bring your passion for movement and meaningful care to Tallgrass Creek, part of Erickson Senior Living, is seeking a patient-centered Physical Therapist to join our team and make a meaningful impact on the lives of older adults. In this role, you’ll deliver evaluations and personalized treatment across a full continuum of care—all within one vibrant, on-site campus. You’ll have the unique opportunity to expand your clinical expertise while building lasting relationships with residents. Requirements

• Must possess a valid and current state Physical Therapy license.
• Degree and training must meet current Medicare and state requirements.
• Ability to document services accurately and on time.
• Specific state regulations and requirements may be applicable. These regulations take precedence over the requirements outlined in the job description.

Nice To Haves

• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Deliver evaluations and personalized treatment across a full continuum of care.
• Expand clinical expertise while building lasting relationships with residents.
• Focus on quality care, working alongside nursing, physicians, and rehab colleagues to help residents maintain mobility, independence, and quality of life.
