The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately. Key functions include conducting thorough physical therapy evaluations to assess patient needs and developing individualized treatment plans, administering safe and effective therapy treatments with appropriate documentation, and reassessing and adjusting treatment plans to align with patient progress. The therapist is also responsible for providing clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information, as well as educating patients and their families on treatment plans, therapeutic exercises, and post-treatment care. Safe operation of therapy equipment and maintaining a clean, organized, and safe environment are essential. Collaboration with referring physicians, peers, and interdisciplinary team members is required to ensure continuity and quality of patient care. The role also involves demonstrating population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs, maintaining compliance with facility policies (including safety, infection control, and risk management procedures), adapting professionally and effectively to changes, performing other assigned duties, maintaining regular and reliable attendance, and complying with all policies and standards.
