About The Position

We are seeking a skilled Physical Therapist (PT) to join our team in providing exceptional rehabilitation services to residents. As a PT, you will assess, plan, and implement personalized therapy programs to enhance mobility, strength, and overall quality of life for patients in our skilled nursing and rehab facility.

Requirements

• Current Physical Therapist license in New York State (NYS).
• Strong assessment and therapeutic skills.
• Ability to work effectively in a multidisciplinary team.
• Excellent communication and interpersonal skills.
• Passion for patient care and improving quality of life.

Responsibilities

• Conduct assessments to determine physical therapy needs and goals.
• Develop and implement individualized treatment plans for residents.
• Provide hands-on therapy to improve strength, balance, and mobility.
• Collaborate with the healthcare team to optimize patient care.
• Document progress and modify treatment plans as needed.
• Educate residents and their families on techniques for improving daily function.
• Adhere to infection control practices and regulatory standards.
