About The Position

Flagstar Rehab is now staffing Physical Therapists for their Rehab Department in state-of-the-art Skilled Nursing Facilities (SNFs) throughout NYC and Surrounding Areas. As a Physical Therapist (PT) at Flagstar Rehab, you will work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education. Your goal will be to improve the physicality and cognition of patients, positively impacting their ability to function in their daily lives. Flagstar Rehab offers the opportunity for Direct Employment (W-2) with their agency or to work as an Independent Contractor (1099), with assistance provided for setting up a corporation free-of-charge.

Requirements

• Current NYS PT License and Registration
• CPR Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
