About The Position
BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for an opportunity performing home health visits with our Hudson County Visits team. This office works with adult and geriatric clients on a per visit basis in territories throughout Hudson County. Immediate opportunity in southern Hudson County (Jersey City / Bayonne) area. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Requirements
- Current New Jersey Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Strong communication, organizational, and clinical assessment skills
- Ability to work independently and manage a flexible schedule
Nice To Haves
- EMR experience preferred
Responsibilities
- Perform home visits in designated geographic territories
- Complete comprehensive physical therapy evaluations
- Assess mobility, strength, balance, gait, transfers, endurance, and safety risks
- Develop individualized treatment plans to improve function and independence
- Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions
- Collaborate with nurses and other clinicians as part of a multidisciplinary care team
- Educate patients and caregivers on exercises, safety, and disease management
- Document patient care using a user-friendly electronic medical record system
