About The Position

Argus Home Health Care has been providing a wide range of medical services since 1990, specializing in home healthcare with expertise and professionalism. The mission is to offer quality care in the home environment, focusing on preventative and restorative medical care with compassion and respect. This position involves providing physical therapy services in clients' homes as prescribed by a physician, offering consultation and education to agency staff, and working under agency policies and client care plans. The role reports to the Argus of Colorado administrator or designee and supervises assigned personnel.

Requirements

Active Colorado PT license
Graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) or any accrediting body approved by the U.S. Office of Education
Valid Driver's License & clean Motor Vehicle Record (MVR)

Nice To Haves

Experience in Home Health preferred, but not required

Responsibilities

Conduct initial and ongoing evaluations, including OASIS assessments.
Develop and update care plans with the physician and care team.
Identify barriers, set goals, provide treatment, education, and equipment.
Coordinate services and plan discharge.
Evaluate care effectiveness.
Record evaluations, treatments, and client responses promptly.
Document conference outcomes and quality assurance data.
Attend care conferences, staff meetings, and in-service programs.
Provide education and consultation to clients, families, and staff.
Supervise aides, therapy assistants, and paraprofessionals.
Maintain professional knowledge through education, professional organizations and workshops.
Perform other related duties as needed.
