About The Position

Blue Stone Therapy is seeking a Physical Therapist to join our rehabilitation team at Mount Olivet Care Home in Minneapolis. This role involves providing individualized physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting, aiming to help them regain mobility, improve function, and maximize their independence. Requirements

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license (or eligibility for licensure) in Minnesota

Responsibilities

• Evaluate patients and develop individualized plans of care
• Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, pain management, and functional independence
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to achieve the best patient outcomes
• Supervise Physical Therapy Assistants in accordance with state licensure requirements
• Complete timely, accurate documentation supporting quality patient care and regulatory compliance
