About The Position

Pathways PACE is excited to have a brand-new Greeley location, and we're looking for a Physical Therapist who wants to help shape the future of care for older adults in Northern Colorado. This isn't just another PT position. It's an opportunity to join a founding clinical team, build strong interdisciplinary relationships, and make a lasting impact on the lives of older adults by helping them remain independent and living safely in their own homes. Requirements

  • Graduate of an accredited entry level program in Physical Therapy
  • Active Colorado Physical Therapist license
  • Current CPR certification
  • Valid Colorado driver's license and proof of insurance
  • One year working with elderly population, home care or geriatric experience

Nice To Haves

  • Ability to work effectively with culturally, economically, and educationally diverse populations
  • Experience working with interdisciplinary teams
  • An interest in preventative, community-based care

Responsibilities

  • Build lasting relationships with participants and their families.
  • Evaluate functional abilities and develop individualized treatment plans.
  • Recommend home modifications and adaptive equipment that improve safety and independence.
  • Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
  • Help participants maintain meaningful daily activities and improve their quality of life.
  • Play an important role in launching a new PACE program and helping establish best practices.
  • Become part of a team that follows participants over time, allowing you to truly see the impact of your care.
