About The Position

Join Iowa Home Care as a Physical Therapist and make a meaningful impact by delivering care where people feel most comfortable — at home. In this role, you will provide in-home evaluations, treatments, and education to help patients improve strength, mobility, safety, and independence. You will collaborate with a dedicated interdisciplinary team, including nurses, occupational therapists, and speech therapists, to ensure comprehensive, patient-centered care. We offer both Full-Time and Part-Time positions.

Requirements

Licensed to practice as a Physical Therapist in the state of Iowa.
Minimum of two years of experience in physical therapy
Valid driver’s license with reliable transportation.

Nice To Haves

Community/home health experience preferred.

Responsibilities

Conduct comprehensive in-home physical therapy evaluations
Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety
Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management
Educate patients and caregivers on home exercise programs, safety strategies, and equipment use
Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care
Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards
Recommend adaptive equipment and home modifications when appropriate
Monitor patient progress and adjust treatment plans as needed
Communicate changes in condition or concerns to the interdisciplinary care team
Promote independence and ongoing improvement through compassionate, patient-centered care
