About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Prince William County. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful impact by helping patients restore independence and improve their quality of life through personalized, one-on-one home-based care. The role focuses on providing individualized treatment plans to help patients regain strength, improve mobility, restore balance, and rebuild confidence in the comfort of their own homes, avoiding the constraints of crowded outpatient clinics.

Requirements

Active Virginia Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky

Responsibilities

Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
