About The Position

Neulife Rehabilitation of Kentucky is seeking a skilled, compassionate, and dedicated Physical Therapist (PT) to join our team in Bowling Green, Kentucky. The Physical Therapist will provide comprehensive evaluation and treatment services to individuals with physical impairments, disabilities, and functional limitations. The successful candidate will develop individualized treatment plans, provide evidence-based therapeutic interventions, monitor patient progress, and collaborate with patients, families, caregivers, and other healthcare professionals to promote functional independence and quality of life.

Requirements

  • Current and valid Kentucky Physical Therapist license, or eligibility for Kentucky licensure.
  • Successful completion of the academic and clinical requirements of a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • Successful completion of the National Physical Therapy Examination (NPTE) as required for Kentucky credentialing.
  • Successful completion of the Kentucky Physical Therapy Jurisprudence Examination as required by the Kentucky Board of Physical Therapy.
  • Must satisfy all applicable Kentucky Board of Physical Therapy credentialing requirements.
  • Doctor of Physical Therapy (DPT) degree or other educational credential that satisfies applicable Kentucky licensure requirements.
  • Strong clinical assessment, treatment-planning, communication, and interpersonal skills.
  • Ability to work effectively as part of an interdisciplinary healthcare team.
  • Commitment to providing compassionate, patient-centered care.

Nice To Haves

  • Previous rehabilitation, skilled nursing, long-term care, outpatient, developmental services, or related healthcare experience preferred.

Responsibilities

  • Conduct comprehensive physical therapy evaluations to assess patients' physical, functional, and mobility needs.
  • Develop individualized treatment plans based on evaluation findings, patient goals, clinical judgment, and applicable standards of practice.
  • Provide therapeutic exercises, functional training, mobility training, manual therapy, and other appropriate physical therapy interventions.
  • Monitor and document patient progress and modify treatment plans as clinically appropriate.
  • Establish measurable treatment goals and work with patients to achieve optimal functional outcomes.
  • Educate patients, families, and caregivers regarding exercises, treatment techniques, mobility, safety, and strategies to promote independence.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, direct-care staff, families, caregivers, and other healthcare professionals to coordinate comprehensive patient care.
  • Maintain timely, accurate, and complete clinical documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
  • Follow applicable federal and Kentucky laws, regulations, professional standards, organizational policies, and payer requirements.
  • Maintain patient confidentiality and comply with applicable privacy and documentation requirements.
  • Participate in interdisciplinary meetings, care planning, quality-improvement activities, and other activities supporting positive patient outcomes.
