About The Position

Evaluates, plans, and implements specific treatment programs for individual patients according to the principles and practices of physical therapy. Functions as an interdisciplinary team member providing patient assessment and reassessment, direct patient care, and individualized care planning and implementation.

Requirements

• Bachelor’s degree from an accredited physical therapy program.
• Licensure and registration in Arizona.
• CPR certification.
• Excellent customer service skills.
• Strong interpersonal skills to interact positively and effectively with patients, families, and physicians.

Nice To Haves

• Six months of experience preferred.

Responsibilities

• Assess patients’ physical therapy needs promptly and complete initial evaluations according to department policies and procedures; document care efficiently and effectively.
• Develop and maintain functional, measurable short- and long-term treatment goals for each patient; reevaluate and modify treatment programs as needed.
• Integrate information, prioritize needs, and involve patients and families in the planning process.
• Educate patients, caregivers, and other appropriate personnel about the patient’s status and recommendations.
• Use the most appropriate treatment methods while providing the highest level of care.
• Maintain and upgrade professional skills through membership in relevant organizations, educational workshops and conferences, self-study, and in-services.
• Perform other related job tasks or responsibilities as assigned.
