About The Position
H2 Health Prime Living is seeking a Physical Therapist to provide 1:1 patient care within senior living communities, specifically at The Gentry Park communities in Orlando. This is a full-time position with a flexible schedule, offering a supportive and inspiring environment for clinicians. H2 Health is a clinician-led and patient-focused organization that welcomes both new graduates and experienced PTs looking for a meaningful career change.
Requirements
- Must have a degree from an accredited Physical Therapy program.
- An active or eligible state license to practice as a Physical Therapist (PT).
- New graduates are welcome.
Nice To Haves
- Outpatient experience is a plus.
Responsibilities
- Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
- Design and implement personalized treatment plans aimed at restoring function and enhancing quality of life.
- Document patient progress accurately using EMR.
- Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
- Contribute to a positive, clean, and professional clinic environment that reflects H2 Health’s culture of excellence.
