Requirements
- Active Kansas Physical Therapist license in good standing
- Physical Therapy License
- Clinical documentation
- Physical & Rehabilitation Medicine
- Long-term care facility experience
Nice To Haves
- SNF or long-term care experience preferred
- Familiarity with documentation systems a plus
Responsibilities
- Evaluate residents’ functional abilities and therapy needs
- Create and implement individualized treatment plans
- Deliver skilled physical therapy services per physician orders
- Document treatments and resident progress accurately and timely
- Collaborate with our interdisciplinary care team
- Educate residents, families, and caregivers
- Ensure compliance with all regulatory and facility standards
