About The Position
Home Health Links is hiring Physical Therapists (PTs) for Start of Care (SOC) visits only across Los Angeles and Ventura County. We’re a therapist-owned home health agency with over a decade of leadership in Southern California—and we use AI-driven OASIS technology that cuts documentation time from 90 minutes to about 15 minutes, so you can focus on patient care, not paperwork. Enjoy flexible scheduling, patients close to home, and strong clinical support. We welcome both experienced clinicians and new grads.
Requirements
- Complete Start of Care (SOC) assessments for new patients
- Degree from an accredited Doctorate of Physical Therapy (DPT) program.
- Current and unrestricted CA PT license; or ability to apply
- Excellent Communication Skills

- Home Health experience preferred; not required
Responsibilities
- Assess the therapeutic and rehabilitative status of the patient
- Develop an appropriate treatment plan for each patient; update and modify treatment plan on an ongoing basis.
- Ensure high-quality patient care
- Use clinical reasoning in treating and planning, implementing, and monitoring patient progress
- Maintain records with accuracy and discretion
