Requirements

Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing.
If operating company vehicle you must be twenty-one (21) years of age and meet the MVR organization standards.

Responsibilities

Provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician’s orders.
Provide leadership and supervision of physical therapy services to other physical therapists, assistants and aides.
Evaluate and treat clients, communicate with families, physicians and other health team members and maintains appropriate documentation of services and complying with State and Federal regulations.
Participates in on-going agency meetings and conducts in-service and training for agency staff on ongoing basis.
Accurately screens and evaluates clients and provides PT upon written order of physician within twenty-four (24) hours of referral.
Prepares and maintains treatment records with evaluation and progress notes.
Supervise physical therapy assistants and/or physical therapy aides in direct client care and client-related activities according to State and Federal requirements. Counter-signs documentations per regulations.
Participates in client care planning conferences, initial conferences, for residents on physical therapy programs, and when requested by nursing and/or therapy department.
