Requirements

  • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing.
  • If operating company vehicle you must be twenty-one (21) years of age and meet the MVR organization standards.

Responsibilities

  • Provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician’s orders.
  • Provide leadership and supervision of physical therapy services to other physical therapists, assistants and aides.
  • Evaluate and treat clients, communicate with families, physicians and other health team members and maintains appropriate documentation of services and complying with State and Federal regulations.
  • Participates in on-going agency meetings and conducts in-service and training for agency staff on ongoing basis.
  • Accurately screens and evaluates clients and provides PT upon written order of physician within twenty-four (24) hours of referral.
  • Prepares and maintains treatment records with evaluation and progress notes.
  • Supervise physical therapy assistants and/or physical therapy aides in direct client care and client-related activities according to State and Federal requirements. Counter-signs documentations per regulations.
  • Participates in client care planning conferences, initial conferences, for residents on physical therapy programs, and when requested by nursing and/or therapy department.
