Requirements

Doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university.
Current license as a physical therapist in the State of Wisconsin.

Responsibilities

Plan, develop, organize, implement, and evaluate physical therapy programs and activities in accordance with current rules, regulations, and guidelines for a skilled nursing facility.
Participate in discharge planning, development and implementation of patient care plans, and patient assessments.
Maintain treatment records, patient files, and progress notes.
Participate in the development and implementation of patient assessments and care plans, including quarterly reviews.
Collaborate with and supervise PTAs.
