The Physical Therapist (PT) is responsible for evaluating and assessing the needs of referred patients in a home care environment, developing treatment plans, and providing therapy services in collaboration with physicians and case managers. This role allows for creativity and latitude in performing various tasks.
Requirements
- Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation
- Current and unencumbered license to practice as a Physical Therapist within the state of employment
- Two (2) years of appropriate experience as a Physical Therapist
- Demonstrates good verbal and written communication and organization
- Possess and maintain valid CPR
- Must have obtained in-person with a practical skills evaluation as part of the final test
- Access to reliable transportation

- Community/home health experience is preferred
