About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Hyattsville, Mount Rainier, Brentwood, New Carrollton, and surrounding Prince George's County communities. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful impact daily. As a Home Health Physical Therapist, you will provide personalized, one-on-one care in patients' homes to help them regain strength, improve mobility, restore balance, and rebuild confidence, enabling them to return to work, live without pain, play with family, and regain independence. The role emphasizes meaningful therapy, lasting relationships, and exceptional patient outcomes, distinct from crowded outpatient clinics.

Requirements

Active Maryland Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus

Responsibilities

Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, social workers, and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
