About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist (PT) to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Dr Palmdale and the surrounding areas. As an independent contractor, you will have the flexibility to control your own schedule, work in preferred coverage areas, and determine your own compensation based on the number of patients treated. FeldCare Connects provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. Clinicians will observe, record, and report patient responses to treatment and changes in condition, determining their own plan of care. Documentation is made easy through an intuitive Electronic Medical Record system, with many clinicians completing documentation during the visit.

Requirements

• Ability and enthusiasm in working with all ages.
• Current Physical Therapist license issued by the state Board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using the Electronic Medical Record system.
