About The Position
H2 Health Prime Living is seeking a Physical Therapist to provide 1:1 patient care within senior living communities, specifically at Hearthstone Nona Lakes in Orlando. This is a full-time position with a flexible schedule, offering a supportive and clinician-led environment for career growth and making a difference in patients' lives. The role is suitable for both new graduates and experienced PTs looking for a meaningful change.
Requirements
- Must have a degree from an accredited Physical Therapy program.
- An active or eligible state license to practice as a Physical Therapist (PT).
- New graduates are welcome.
- Outpatient experience is a plus, but we value passion and a commitment to patient care.

Responsibilities
- Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
- Design and implement personalized treatment plans aimed at restoring function and enhancing quality of life.
- Document patient progress accurately using EMR.
- Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
- Contribute to a positive, clean, and professional clinic environment that reflects H2 Health’s culture of excellence.
