About The Position
Linley Park Post Acute is a beautiful 88-bed skilled nursing and rehabilitation facility located in the heart of Anderson, SC. We are a family-oriented team dedicated to making a meaningful impact on our residents’ lives every day. As we continue to grow, we’re expanding our in-house therapy team to deliver the highest quality care and help residents achieve their greatest level of independence.
Requirements
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Active South Carolina PT license in good standing
- Familiarity with PointClickCare and Casamba preferred
- Strong clinical, leadership, and documentation skills
Nice To Haves
- Experience in skilled nursing or rehab setting preferred
Responsibilities
- Evaluate residents within required timeframes and develop individualized treatment plans
- Provide therapy services in accordance with physician orders and care goals
- Document evaluations, treatments, and progress notes per regulatory standards
- Supervise PTAs, aides, and students in compliance with state practice act
- Collaborate with interdisciplinary team members on resident care plans
- Participate in care conferences, utilization review, and rehab meetings
- Educate residents, families, and caregivers on treatment plans and discharge needs
- Assist with durable medical equipment (DME) recommendations
- Provide staff training and in-service education
- Maintain compliance with all state, federal, and payer regulations
