About The Position

Goodwin Home Health is seeking a dedicated full-time Physical Therapist to join their vibrant team. This role involves delivering quality care at the patient's home, conducting detailed patient assessments, devising individualized Plans of Care, providing expert physical therapy as per physician directives, collaborating with the broader team for optimal patient outcomes, and streamlining communication with referring physicians while maintaining thorough and timely documentation.

Requirements

• Graduate of an accredited school of physical therapy.
• Active Physical Therapy License in the Commonwealth of Virginia.
• Prior home health care experience.
• Current CPR certification.
• Possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.

Responsibilities

• Spearhead detailed patient assessments to devise individualized Plans of Care.
• Offer expert physical therapy in the Home Health sphere as per physician directives.
• Collaborate closely with the broader team, ensuring optimal patient outcomes.
• Streamline communication with referring physicians, maintaining thorough and timely documentation.
