About The Position
Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are proud of our small-town feel, strong community connections, and commitment to providing compassionate, high-quality care to our residents. We are seeking a motivated Physical Therapist (PT) to join our team on a PRN basis. If you’re passionate about helping patients regain mobility and independence, we’d love to connect with you.
Requirements
- Bachelor’s degree required; advanced degree preferred
- Active, unencumbered Physical Therapist license in South Carolina
- Strong communication and clinical skills
- Ability to work independently and as part of a team
Nice To Haves
- Experience in a long-term care or rehab setting preferred
- Familiarity with PointClickCare (PCC) and Casamba preferred
Responsibilities
- Perform patient evaluations and develop individualized treatment plans
- Deliver therapeutic interventions to improve strength, balance, and mobility
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary teams to support patient outcomes
- Educate residents and families on therapy techniques and recovery goals
