About The Position
Indigo Manor Nursing and Rehab is seeking a skilled and compassionate Physical Therapist to join its interdisciplinary team. The therapist will work with patients, families, and healthcare professionals to develop and implement personalized plans of care that address each patient’s needs and goals. Responsibilities
- Conduct thorough patient evaluations and assessments, identifying strengths, weaknesses, and potential for improvement.
- Develop and implement individualized treatment plans using evidence-based techniques and best practices.
- Provide hands-on physical therapy interventions, including exercises, stretches, and modalities.
- Collaborate with occupational therapists, speech therapists, nurses, and other healthcare professionals to provide coordinated care.
- Monitor patient progress, adjust treatment plans as needed, and communicate with patients, families, and healthcare team members.
- Maintain accurate and timely records in accordance with regulatory and professional standards.
