When you join the growing BILH team, you're not just taking a job, you’re making a difference in people’s lives. This role involves coordinating patient care for an assigned caseload, including appropriate delegation to and supervision of PTA staff, handling patient condition changes, and completing OASIS time points. The position accepts accountability for patient satisfaction scores and quality of care outcome measure results. The Physical Therapist will complete and document a physical assessment and history, develop a patient-specific plan of care incorporating therapeutic, preventive, and rehabilitative actions, and include the patient and family in the planning process. They will initiate the plan of care, make necessary revisions, and administer prescribed medications and treatments. The role also involves counseling patients and families, respecting cultural beliefs, providing health care instructions, instructing and supervising home health aides, managing episodes of care, and identifying discharge planning needs. Communication with physicians, team members, and community providers is essential for coordinating care. Proficiency in OASIS completion and timely documentation are required. Additional duties include participating in weekend/holiday rotations, ensuring necessary equipment and services are available, and participating in in-service programs and precepting other physical therapy personnel.
