About The Position

Aspire Home Healthcare is seeking a full-time Physical Therapist to provide in-home care to patients in Delaware, OH and nearby communities. As a Physical Therapist (PT), you'll evaluate patients, develop and implement individualized treatment plans, and collaborate with our rehab team to deliver high-quality care in a home health setting.

Requirements

• Active PT license in Ohio
• Current TB test and CPR certification
• Valid driver’s license and auto insurance
• Ability to attend orientation in Dublin, OH

Nice To Haves

• 1+ year of PT experience
• OASIS and WellSky/Kinser experience

Responsibilities

• Evaluate patient condition and functional level
• Develop and implement personalized treatment plans
• Supervise PTAs and HHAs per plan of care
• Provide in-home therapy and adaptive equipment guidance
• Maintain accurate and timely documentation
• Educate patients and families on home exercise programs
• Submit notes and visit documentation promptly
• Adhere to HIPAA confidentiality standards
