About The Position

Navitas Healthcare, LLC is seeking a Physical Therapist – Skilled Nursing Facility for an exciting Travel or Local job in Phoenix, AZ. This role involves evaluating patients, developing individualized treatment plans, and providing therapeutic exercises and rehabilitation services to patients recovering from injury, illness, or surgery. The Physical Therapist will monitor patient progress, adjust treatment plans as needed, and educate patients and family members on therapy programs. Collaboration with physicians, nurses, and the interdisciplinary healthcare team is essential for coordinating patient care. Maintaining accurate documentation of patient treatments, progress notes, and discharge planning is also a key responsibility. The goal is to assist patients in improving strength, balance, flexibility, mobility, and functional independence, while ensuring compliance with facility policies, patient safety standards, and healthcare regulations. Participation in care planning meetings and contributing to quality patient outcomes are expected.

Requirements

  • Active Physical Therapist (PT) license in the state of Arizona or compact eligibility
  • Graduate of an accredited Physical Therapy program
  • Current CPR/BLS certification preferred
  • Minimum of 1 year of recent Physical Therapist experience preferred
  • Skilled Nursing Facility (SNF) or rehabilitation experience required
  • Strong communication, patient care, and clinical assessment skills

Responsibilities

  • Evaluate patients and develop individualized physical therapy treatment plans
  • Provide therapeutic exercises, mobility training, and rehabilitation services to patients recovering from injury, illness, or surgery
  • Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals
  • Educate patients and family members on therapy programs, exercises, and injury prevention techniques
  • Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care
  • Maintain accurate and timely documentation of patient treatments, progress notes, and discharge planning
