Requirements

Graduate of an accredited Physical Therapy program
Current and active Physical Therapist license
Current CPR certification
Valid driver’s license and reliable transportation
Current auto insurance

Nice To Haves

Minimum 1 year of Home Health experience preferred
HomeCare HomeBase experience preferred

Responsibilities

Perform and document initial evaluations to identify patient needs and establish treatment goals.
Develop and implement individualized treatment plans to maintain, improve, or restore physical function.
Evaluate patient progress and modify treatment plans to maximize outcomes.
Educate patients and families on therapeutic techniques and care plans.
Apply infection control and universal precautions while delivering patient care.
Ensure continuity and quality of care through timely and accurate documentation.
Collaborate effectively with physicians, clinicians, and interdisciplinary care teams.
Promote agency values, patient-centered care, and company policies.
Maintain documentation in accordance with company standards and regulatory requirements.
