About The Position

FOX Rehabilitation is expanding in Gluckstadt, MS, and surrounding areas, seeking licensed Physical Therapists to provide one-on-one care to older adults in their homes or senior living communities. The role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position offers autonomy, flexibility, and the ability to manage one's own schedule, fostering strong patient relationships. Clinicians may also utilize telehealth and virtual care options for increased flexibility and patient support. This is a high-impact role for Physical Therapists aiming to positively influence the lives of the communities served.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences or Senior Living Communities.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions focused on patient wellness, longevity, mobility, and functional independence.
Document effectively within FOX’s EMR system according to regulatory standards.
Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff on safety, function, and aging in place.
Provide telehealth / virtual care as appropriate to enhance access and flexibility.
