About The Position

We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. The mission of this role is to deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Requirements

  • Active Michigan PT license (in good standing)
  • Strong clinical judgment
  • Comfort working independently in the field
  • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication skills
  • Strong documentation skills
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver’s license
  • Comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health PT scope
  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
  • Strengthening, functional mobility, and home safety assessment/modification education
  • Patient/caregiver coaching with practical home programs

Nice To Haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper clinical experience in high-signal PT populations
  • Experience supervising/mentoring PTAs and collaborating on care plans
  • Strong organization and time management skills

Responsibilities

  • Deliver excellent home health care to patients in Detroit.
  • Set the clinical bar, the documentation standard, and the referral-source trust for the market.
  • Carry a full, high-quality caseload, ramping up to approximately 28 productivity points per week at target visit quality.
  • Own patient outcomes, driving low avoidable-hospitalization rates and strong patient-experience scores.
  • Participate in case conferences and team huddles to improve quality.
  • Act as a referral-source ambassador, ensuring responsiveness and excellence to encourage future referrals.
  • Set the standard for charting, cadence, and judgment as a template for new hires.
  • Manage the patient's therapy plan, progression, and outcomes as a case manager.
  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute clear plans of care including gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment as needed.
  • Drive multidisciplinary coordination by communicating changes in condition and aligning on orders.
  • Manage treatment and outcomes, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) in a defensible, efficient, and consistent manner.
