Requirements

Current and unrestricted California Physical Therapist (PT) License required.
Strong clinical, communication, and time-management skills.
Ability to work independently while contributing to a team-oriented culture.
Passion for delivering exceptional patient-centered care.

Nice To Haves

Previous experience in a skilled nursing facility, rehabilitation, or post-acute setting preferred.

Responsibilities

Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy services to post-acute and skilled nursing residents.
Improve strength, mobility, balance, endurance, and functional independence.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.
Educate patients and families on treatment goals and home exercise programs.
Maintain accurate, timely, and compliant clinical documentation.
